UIPA LOG for FY 2015

June 30, 2014 is the last day to record requests on the UIPA Record Request Log for FY 2014.  While agencies should continue to use their FY 2014 Log to track how requests were resolved, they should also start a new FY 2015 Log for requests received from July 1, 2014 through June 30, 2015.  Annual totals for FY 2014 must be provided by each agency to the overall UIPA Coordinator for each state department/organization, who will subsequently upload all totals onto the “OIP Master UIPA Record Request Year End Log for FY 2014” at data.hawaii.gov, by August 30, 2014.

Attached is the FY 2015 Log form for all state and county agencies to use beginning July 1, and it can also be found on OIP’s Log training page at https://oip.hawaii.gov/laws-rules-opinions/uipa/uipa-record-request-log/. (Please be sure to use the worksheet for the Form Log, and not the Sample Log, when starting the agency’s FY 2015 Log.)  Additionally, the training video and power point materials have been revised and can be found on the Log training page.
