Office of Information Practices

The State Office of Information Practices (OIP) has posted on its website two revised UIPA forms:  “Request to Access a Government Record” and “Notice to Requester.”  Thanks to the suggestions from Martha Im at the Department of Commerce and Consumer Affairs, OIP reviewed the forms and made revisions that should make it easier for agency personnel and the public to understand.  If you have constructive suggestions for improvements, OIP wants to hear from you, too.
